On the day of your admission to Drake Center, a member of our admissions team will come to your room to review and help complete the necessary paperwork; or, if you prefer, a family member can visit the admitting office to handle this.
Please have the following with you when you meet with the admissions team:
- Insurance, Medicare or Medicaid identification card
- Copies of Living Will and Durable Power of Attorney for Health Care (if completed)
- Name, address and phone number of primary care physician
Our nursing staff will then provide you and your family an orientation to Drake Center's services and to your particular unit. You also will be given a personalized Patient and Family Collaborative Care Notebook, which includes information on your rights and responsibilities as a patient, Drake's services, tips on adjusting and coping, and customized details on your individual condition, care, therapy and discharge plan.
Once you are admitted, you will be assigned a staff of experts assembled to meet your unique goals and diagnosis. As important members of this team, you and your family will be actively involved in making decisions about care. You also will be assigned a registered nurse case manager and social work-discharge planner who will be available to answer questions about services and progress throughout your stay and to assist with discharge preparation.
