Transitional Skilled Nursing Care
Our Transitional Skilled Nursing Care program provides short-term skilled medical and rehabilitation care to bridge the gap between hospital-level and a more independent level of care.
Ideal patients are:
- Respiratory patients who have been weaned from a ventilator, but still have a tracheostomy tube
- Wound care patients with extensive wounds requiring special treatments and nursing care
- Medically complex patients who no longer need daily physician visits, but do need daily nursing assessment and care
- Orthopedic patients who are recovering from surgery or fractures
Most patients stay in this program about three weeks where they benefit from:
- An individual approach to therapy
- Medical oversight by an interdisciplinary team of physicians, therapists, nurses and social workers
- Easy access to a dedicated therapy gym
- 24/7 nurse coverage
- An on-staff recreational therapist to help them regain the ability to participate in social and recreational activities
